Application Forms

ONLINE PROGRAMS
If you are seeking ODA confirmation of your exempt status as a purely online program to meet federal rules, please submit the Initial Approval Request form and supplementary materials that fully describe your program(s) and any requirements, fees and policies affecting Oregon students. ODA staff will contact you if additional information is needed to determine your exempt status. You will be billed for a $250 fee for review of your application and verification of your status. Please allow at least three weeks for processing of your exemption. If you have additional questions about this form or the process, please contact ODA *only* by e-mail, as we do not have sufficient staff to respond to phone inquiries about online programs. Allow at least one week for response.

PRACTICUM PROGRAMS
Please submit an Initial Approval Request form and supplementary materials for each degree program that includes a practicum, clinical, internship, externship or other experiential learning component for students in Oregon. Supplementary materials should fully describe the structure and function of the practicum, as well as the qualifications of faculty and staff of your school and of the partners at the site in Oregon where students will be participating in experiential activities or courses for credit. ODA staff will contact you if further information or a site visit is required to process your application. You will be billed for a $500 fee for review of each specific program application and authorization of your practicum for a two-year period. Please allow 3 months for processing of your authorization.

Partial Degree Programs
Please submit an Initial Approval Request form and supplementary materials that fully describe the courses and components of your program that take place in Oregon, as well as the qualifications of faculty and staff of your school and of the partners at the site in Oregon where students will be participating in experiential activities or courses for credit. ODA staff will contact you if further information or a site visit is required to process your application. You will be billed $500 for review and authorization for a two-year period to offer courses for college credit. Please allow 3 months for processing of your authorization.

SCHOOLS WHO WISH TO OFFER NEW DEGREE PROGRAM(S) IN OREGON
Please submit both one General School Application Form and a Specific Program Application Form for each program you will offer in Oregon, complete with any supplemental information requested in the application forms. These files should be submitted in an electronic format, such as PDF files, on a disc or flashdrive, and sent to the ODA office via postal or delivery services. We are unable to accept electronic submissions via email at this time, due to security concerns. ODA staff will contact you if further information, or a site visit, is required to process your application for authorization of a degree program. Upon receipt of your application, you will be billed for the appropriate fees for review of each specific program application and authorization for a two-year period. Please allow 4-6 months for review and processing of your authorization to offer degree program(s) in the State of Oregon.
